Usage Instructions:

1. Insert a new OneTouch Ultra Plus™ test strip into your compatible OneTouch meter.

2. Apply a small blood sample to either side of the test strip.

3. Wait approximately 5 seconds for your blood glucose reading to appear on the meter display.

Storage and Handling:

• Store test strips in a cool, dry place.

• Keep test strips in their original vial; do not transfer them to another container.

• Close the vial tightly immediately after removing a test strip to protect from moisture.

• Use each test strip immediately after removing it from the vial.
